The Disciple Maker Index Survey seeks input from parishioners in the diocese about their experience of the church and their parish. Please read below to learn how to access this survey before it closes on April 4th.
For the past several weeks, the diocese has been seeking feedback from parishioners across the diocese by way of a survey. The Survey named Disciple Maker Index Survey, seeks input from parishioners in the diocese about their experience of the church and their parish. The survey became available on March 2nd and is due to close on April 4th at 11:59PM.
The survey is provided by and will be compiled by the Catholic Leadership Institute. It is made possible by a grant funded by the Lilly Foundation which requires that all parishes and priests within our diocese will have participated in this survey. The purpose of the survey is to give beneficial information to all parishes for future ministry and continued viability.
If you have not already done so, you are encouraged to complete the survey using the link provided in the bulletin. There are just a few more days before the survey will be closed. As of this writing, 36 surveys have been completed online.

ACCESSING THE DISCIPLE MAKER INDEX SURVEY
All responses will be confidential and the parish will only receive information about the community as a whole.
There are two ways you can complete the survey:
1) Online. Use the link below to access the survey: https://portal.catholicleaders.org/dmi/survey/y5tgby
2) On Paper.
· A limited number of printed surveys are available on the tables at the Main Street entrance or parking lot entrance of the church
· Return the completed surveys: (1) to the boxes on these tables, (2) in the collection basket, (3) directly to the rectory. Paper copies must be submitted by April 2nd, 2022
